Privacy Notice Statement & Acknowledgement of Consent


Your privacy is important to us. At Llorens Insurance Agency, Inc., we are in the business of building and maintaining relationships with our clients, so we take your privacy very seriously and will protect it, as we would our own. We understand that all information you provide to us is private. The information you provide to us is kept confidential and is used solely to complete our responsibilities. This privacy policy is provided to you so you will understand what we do with the personal information you provide to us.


Personally Identifiable Information (PII) includes individually identifiable information such as your name, address, telephone number, date of birth, email address, or Social Security number. We are authorized to collect PII from you on behalf of the insurance companies and other entities we represent. We will use PII only to the extent necessary to carry out our authorized functions including but not limited to assisting in obtaining the health insurance or other coverage you are seeking. We protect this information in all formats – electronic, written, and verbal information. Any PII we collect is used to assist you in obtaining health insurance or other coverage as requested by you and to assist you in resolving any issues related to your health insurance or other coverage as requested by you.


PII is only used by or disclosed to those authorized to receive it or view it. If you choose to give us PII, we may share this information with 1) CMS (a federal agency) and information will be maintained in a federal Systems of Records and/or 2) insurance companies or other entities so they can issue you a health insurance policy or other coverage and will be maintained in their systems. 


PII is used or disclosed under the following circumstances:

  • Where required by law
  • To you or someone who has the legal right to act on your behalf
  • To your family or friends if you are unavailable to communicate, such as in an emergency
  • To any person you identify as appropriate to receive your PII
  • To the Secretary of the Department of Health and Human Services and related entities
  • To assist you in obtaining health insurance or other coverage
  • To assist you in resolving any issues related to your health insurance or other coverage


PII is given voluntarily. If you choose not to provide us with the PII requested, or not to respond to certain questions, we may not be able to assist you in obtaining health insurance or other coverage you are looking for. If PII needs to be corrected, you may call us at 770.822.1050.


PII is securely destroyed or disposed of in an appropriate and reasonable manner and in accordance with retention schedules.


Protected Health Information (PHI) includes information about your personal health such as your symptoms, test results, diagnosis, treatment, and related medical information. PHI also includes payment, billing, and insurance information. While we do not normally need this information, if you share it with us, we will keep it private, as we would PII. 


We reserve the right to update our privacy practices and the terms of this notice at any time, as allowed by law. This includes the right to make changes in our privacy practices and the revised terms of our notice effective for all personal and health information we maintain and will apply to all PII or PHI created or received prior to this revision. When we make a significant change in our privacy practices, we will change this notice. A current version of our Notice is available under the Resources tab of our website: LlorensInsurance.com. 


We are able to assist you in obtaining health insurance or other coverage at no cost to you because we are compensated by the insurance company, as long as you enroll through us. Compensation rates vary by insurance company. Buying an insurance plan through a broker or agent does not change or increase your insurance plan premiums, copayments, coinsurance, or deductible. We may be eligible to receive commissions, fees, or other compensation from the insurance carrier who pays brokers or agents for providing service to their members. This is being shared in accordance with section 202 of the Consolidated Appropriations Act.


This document also confirms the consent provided by you verbally and documented when your application was completed and acknowledges that you were informed of our role in the application process.


Specifically, you have given us permission to: 

  • Conduct a search for your application on the marketplace
  • Assist you with completing an eligibility application
  • Assist you with plan selection and enrollment
  • Assist you with ongoing account and enrollment maintenance


This consent continues until it is revoked in writing.
